We are IN-Network with: First Choice Health, Medicare, MODA Health, Providence, Regence/BCBS, Motor Vehicle, & Workers Comp.
We do bill out-of-network insurances but we DO NOT bill: OHP, Kaiser, or United HealthCare.
Services that are not billed to insurance are eligible for a "Date of Service" discount rate. A Superbill may be provided for those who wish to self-submit to insurance.
